Sumbitting an edited form, CCFv5 do an INSERT with NULL values instead of UPDATE the editing entry.
Does your form have a field with a name matching the primary key field name in your table ?

You are using Chronoforms only, not Connectivity, correct ?
If yes then it all depends on your "db save" settings, please make sure that the "save under model id" setting is enabled, and set the model id according to your fields names.

Well, if this form is used to save the data using CC then you should NOT have a "db save", but should have a "Connection action", please read the CCv5 tutorials for this!
But if this is a standalone CFv5 form then your config is correct, if this the "db save" is enabled and the "save under mode id" is enabled and the model id is correctly set!
