I need to build a form that will allow me to insert the name of the website and its URL into a table in a database.
When records from the database table are pulled up into a table in an article the name of the website should be the only name showing in the appropriate field, and it needs to be able to activate the URL associated with it when clicked on. There will be other fields for each record in the table within the article, however, how to setup web names with hyperlinks has had me going in circles.
To put it another way, I would like to make a form that would allow me to enter the data into it so that a new record will be created in the table each time the form is submitted. A hyperlink activated by this website name is only one of the fields within the record that will be created by the form.
Can someone please walk me through the method for doing this as you would walk an eight-year-old through something you're trying to explain to him/her. I'm definitely a newbie, and I'm hoping that this can be done without using code.

This can't be done without code since you need to transform the displayed value of your field.
You can check the "DB Multi record loader" under our Tutorials/V4 page, and when you are displaying the field value, instead of writing the field name inside 2 curly brackets:
