I recently upgraded to the latest version from 3.0.
when I click on a form in the back end to preview - i get the "There is no form with this name or may be the form is unpublished, Please check the form and the url and the form management "
the forms are published
I did the sql upgrade
I had them backed up and restored as usual...
If I put them in a content item using the plugin, I can get to the form, but when I hit submit, I get the same error...

ok - figured it out...
we're using sh404 and Squid caching...
you CAN'T have '-' in your form name...
grrr and duh
