Are you sure the plugin is installed and active ?

Also is "Contact" the "alias" of the form ?

Do you have the website cache enabled ? if yes then disable it and test, if it solves the issue then you need to install the System Plugin instead and deactivate the content plugin

it works fine here I think because the site was an upgrade from Joomla 4, on fresh Joomla 5 websites (is your site a fresh one?) it may cause an error

I will release an update soon and post here.
