Upload File Name only Received File not received

Hi,

I have Use Chronoform5,am new to chronoform,my problem is i have received email perfectly but only i have receive file name i didn't get file,here i have attached received email,please give some idea to fix the issue

Have you added the name of the File Upload element into the Attachments box of the Email action?
