After design form I save it and when I open it and save again I lost more then half of the fields. I attach the backup of form. Thanks for help.

The form looks OK to me, I can add a field and is saves OK - what fields are missing?

Hi Bob,
all after field nausea-during. I shoot the pictures:
after-save.jpg - state after saving
edit.jpg - state if I edit the losting fields before saving
after-edit.jpg - the error after close the edit window

The edit Error showed when I edit fields which lost after I save the form.

It's hard to say what is happening. It might be the dashes in the file names, they can sometimes cause problems, but not usually with the form saving. It might be something about the options you are adding; it might be a server setting that is causing a problem.

I suggest that you make a backup of your form and try changing the input names from e.g. nausea-during to e.g. nausea_during and see if that makes a difference.

I try add only fields without character "-" and when I hava 20 fields on form then do the same behavior.
1. I add any next field,
2. save and close form
3. open form again
and now I have the error.

In attachment are two files 1st backup OK and second with error after add next field.

I can add more inputs an save OK. Please see this FAQ - that might be the problem here.
