Form submission shows upload box

Everytime I submit my new working form it sends back to an upload file field:

Nothing out of the ordinary. I've use CF many times for Joomla, this is how I have always done it. For some reason this form keeps wanting me to upload a file😟

Only using AJAX form submission does it seem to work.

I'm not clear what your images show - in the second post there is a rather odd-looking SEF URL that is giving you the 404 response. The File upload form doesn't look like a ChronoForm - is that the default 404 page for your site?

The form images in the first post look OK to me.
