Errors when submitting form

I upgraded to Chronofroms 5.0.4. My client wanted a couple of changes on their Reservation Form. I'm now getting errors but not sure what fields are causing them. I'm including two screen captures.

Hi Joe,

With Development Tool i can see something wierd:

[attachment=0]joomla01.JPG[/attachment]

When I click on the link that blue arrow shows, i can see this:

[attachment=1]joomla02.JPG[/attachment]

I hope it isn't a bug version, I really thought about it, but I'm not sure.

Well for now I had to turn off Sever Side Validation to get it to work. Don't know if that is a bug in the latest version or not.
