I am looking for someone to migrate four old forms from an old Joomla 1.5 site into a newly installed J3.x site running Chronoforms 5. The forms use some javascript to save data to tables and I can't figure out how to do it. There is also one new form that needs to be created. The forms are behind a login on the site so I can't really give you a direct URL to look at right now. but pasted below are screen caps from the two most complicated forms. The names are being pulled in from Community Builder I believe.
