Erro when a receive email

Email missing form fields after submission.

Overview

The email template in the ChronoForms action was not properly configured.
Add and configure an email template within the CF Email action to include all required form fields.

When a submit my form, I receive a email without some field.

My link form is: http://www.lotushoteis.com.br/newsite/index.php?option=com_chronoforms5&chronoform=Contato2

The email that i receive shows:

Ok, i was resolved. The problem was with de email template.
