Instructions on Dynamic Dropdown elements

Please point me to a place where I can get detailed and simple instructions on creating a dynamic drop down element for my form.
No matter where I look, it seems that there are more questions/problems than answers.
I have looked at the demo and cannot understand how it works let alone build one.
Before I tear the rest of my hair out please help :?

Please check the "Events" tab in the dropdown element in the demo form!
