Form disappeared (When I updated to Joomla 3.3?)

Huge fan of Chronoforms5🙂

Used it for a week or two, then I updated my site to Joomla! 3.3 ... not sure if the bug showed up away, but the chronoform5 disappeared completely and now there is just a blank page...
I installed Chronoforms4, it worked (and I still have it, it's just disabled), it's not as spiffy.
I even uninstalled Chronoforms5 and reinstalled it just in case -- still dealing with a blank page.
Component, Module and Plugin are successfully installed.
Test Form also comes out blank.

Any way to get forms to show again via Chronoforms5?

Have you upgraded ChronoForms to the latest release for Joomla! 3,3 - releases for earlier versions may not display on 3.3.

Yeah, I definitely updated to the latest releases (I even re-downloaded from the site last night, uninstalled my old Chronoforms5 and then reinstalled with the new download just in case).

I also suggest testing any of the demo forms (click demos in forms manager) using the "View form" link, do they work or not ?

Bob - I looked at the FAQ and tried "emporarily changing Error Reporting to Maximum in the Site Global Configuration and see if you get a meaningful error message" -- however, there was no meaningful message.
Max - The demos do not work either.

If this means anything: Chronoform4, which had been working for a little, also just stopped working - Now I get a blank page; also for the demos. Error reporting is turned onto Maximum.
I think had an error message for line 11 in chronoforms.php but did not write down the whole message before the entire form disappeared completely.
No site/joomla! updates were made between the time it worked and the time it stopped working.

However - the form not displaying only occurs when I click on "front end view" - it did work in the past, just stopped working.
I have a testing site form DOES displays when I click "test form," this is the error message I see: Notice: "Constant DS already defined in /home1/kcyclist/public_html/components/com_chronoforms/chronoforms.php on line 11"

Sorry for so many messages - just noticed this error message as well:

/public_html/administrator/templates/isis/html/message.php on line 24

Ok, please try to download v5 again and install it OVER yours (do NOT uninstall), check if this helps ?

Max, I just reinstalled and it still does not work.
The form does not appear in either the TEST or the VIEW FORM options.

If anyone can take a quick look, I'd be glad to PM my admin details.😟

Just noticed this "Submit Your Miles (Alias: submit-your-miles)
Component 'com_chronoforms5' does not exist" as well on the module page ...

Hi igcyclist,

The CFV5 forms are OK if you view them without the template (see here for example). So there seems to be a serious problem with your template which is not showing any page content on these pages. There is a menu and some other odds and ends but nothing visible - and no error messages.

You have a very old version of CFv4 installed, please try upgrading to the current release of CFv 4 for Joomla! 3 if you want to use those forms.
