Submit Button Test Form/Live Site

Hello,

I am pretty new at using the program so forgive me if this is a "newbie" question.
I'm trying to create a simple form that allows a user to downloads a pdf file once they enter in the required fields and click on a submit button.
I have the form working correctly in the process I want when I use the "Test Form" option in the Form Wizard.

However, when I copy and paste the HTML code to either an article or module, the submit button is unresponsive.
Is there a step I am missing?

Yes, you can't just copy the Form HTML like that. Use the ChronoForms Plug-in or Module to add your form to an article or module location.
