How do I allow Multiple selections in radio box

Hi,

Wondering how I can allow multiple choices with radio box.

Basically i have a radio box with about 14 options to choose from and i want the person filling out the form to be able to choose all options that apply.

Thanks you're absolutely right. I feel pretty foolish now!

I am trying to replicate a google form and one of the fields appears to be a radio box group. Its Probably a check box group styled to look like radio box.
