Dear sir(s),

I just copied an old form to my new website.
The submit button doesn't work anymore.
I can see the form <form> tags, but they start at the upload file section and end immediately after this section.
So it seems that <form> doesn't include the whole form, only the small upload section.
Please help.

Maybe something got lost in the copy and paste and there are now some broken quotes or tags.
