Form doesnt shows up

Hello,

So i've been making my own forms, but when i want to display it, it wont show up.. the only thing i see is the ''powered by'' line.. i already dragged the html to the on load section.. it then does shows up on:

/index.php?option=com_chronoforms5&chronoform=Contact

But thats not the right url, because when i go over to the contact tab on my menu i only get to see the ''powered by'' line..

Quick update, it does shows up when i click on view form, but when i go over to my contacts tab on my site it doesnt shows up

The contact tab is a menu item of type Chronoforms5 ?

Indeed it is. For some reason it is showing up right now.. i have now idea what happend, but its fixed for now haha
