I am using J! 1.5.1 and have both the component and mambot enabled. However when I include
I tested this in a normal article and the form appears as expected. The other posts I have found either don't give a solution or have an oblique reference to enabling plugins: see here. I am using mod_custom and that has no such setting.

My guess is that mod_custom doesn't support PlugIns (I had a quick look and can't find anything to suggest that it does.)
Please check this FAQ which contains a link to the Content Item Module which does support PlugIns - though you have to find and enable that setting.
