Image not attached or stored in folder

Hello,

I have been reading every available forum post about this subject but sadly i can not get an image uploaded with the form used. It als does not get stored in the designated folder. I have the filefield included in the form, did all the settings in the uploads tab and filled in the attachment files field on the email tab. Still no succes :-( When activating the debug mode i get the follwing report in which it shows that there is not even a attachment field. Hopefully someone can point me in the right direction.

Hi jigsaw,

From the info in you post I don't see any sign that you have file-upload input on the Designer tab? It's not showing up in the debug array.

Hi Bob,

I have included a print screen (previous post) where you can see the element tab (pop-up). This is from the file_upload input which i did fill in. I might have done something wrong there but i can't see what might be wrong.
