PLEASE HELP!

Can someone please explain how to setup the File Upload filed in v5.

I have created a file upload field in my form, and set a File Upload on Submit action. I have the folder created on my server and the path to it.

However, when I complete the form, NOTHING happens, the form is submitted and I receive an email, but the file is not uploaded and does not appear in the folder on the server. Each person filling in the form will be uploading a file with a different file name although they will either be .doc, .xls or image files.

Can someone please provide, step by step instructions on exactly how to achieve this.

Please help this is on a live website and my client is anxious!

I HAVE SOLVED IT!
on the Setup in the submit actions, I had copied the example:

field1:jpg-png

When you need to replace the field1 text with the filed name of the upload filed you used on the form.
