Blank Page ChronoForms V5

I'm not completely new to Chronoforms, and recently installed Joomla 3.2.2, running on PHP 5.4. The forms are currently not showing up in the front end, neither on the standard Joomla template.
When I try 'test form' in the backend, I get this URL: index.php/component/com_chronoforms5/chronoform,Contact/
with a message saying: Form name can NOT be empty! (Form name = Contact, as you can see in the URL)

Could somebody help me out here? Thanks!

It's a problem with SEF URLs but I haven't seen this before with the Test Form link :-( Please try the URL

index.php?option=com_chronoforms5&chronoform=Contact

and see if that works.

I wonder if this is something changed in 3,2,2??

Same issue with index.php/?option=com_chronoforms5&chronoform=Contact
Also tried it when SEF was turned off, same thing.

I have the same issue, just get a blank page:
