CF V5 - Events disappear once form is run on front-end

I am running CF v5 and creating a donations form. The piece in question I am having challenges with is the portion to select an amount. For this function I am using a radio box group. I only want one option selected (max). I have 5 options, the first 4 are various amounts and the 5th choice is Other. If a user selects Other, then another text field (Other Amount) should appear where they can enter their own amount.

On the radio box group, in the Events tab, I filled out various events. Five total Events, one for each option. Options 1-4 when selected Hide the Other Amount field, and option 5 shows the Other Amount field.

When I then test out my form on the front-end. Only some of the Events function correctly. After checking my Events again, some of the Events I entered and saved, have disappeared.

Are there any know issues with this behavior?

Another user has reported that the events disappear, but I couldn't reproduce the problem, please send me a sample form backup with the problem, using the "Contact us" page.

When I go to the 'Contact Us' page there is no attachment field visible. Did you mean for me to enter the code in the Message text area?
