Do you hear it? Yes or No

Hi, I've a question…
I'd like create a form with dropdown element with a question like this "do you hear this sound?", and display a button play to listen the sound. Is it possoble?

I've never tried this but it should be possible. You can use JavaScript to hide and display the buttons or icons and to play a sound when they are clicked.
