In the form SHOW HTML action the settings are:
Republish form data - Yes
Load Security Token - Yes
Load Keep Alive - Yes
Curly brackets replacer - Yes
Normally the email submission date format is 2013-07-21, but over time. the following changes occurred without my doing anything (as far as I am aware) to cause them:
July 8: Booking Request for {firstname} {lastname} for {checkin} to {checkout}
July 12: Booking Request for John Brown for 17/july/2013 to 20/july/2013
July 16: Booking Request for John Brown for july 26, 2013 to july 29, 2013
July 22: Booking Request for John Brown for August 14, 2013 to August 14, 2013
July 27: Booking Request for John Brown for 07/27/2013 to 07/29/2013
July 28: Booking Request for John Brown for 8.16th, 2013 to 8.18th, 2013
Can you say:-
(1)What is the possible cause, and how I may fix it?
(2)How to set the email submission date format to "August 14, 2013"

I've no idea. Where does your email submission date come from? This range looks as if there is something related to browser date locales rather than anything in ChronoForms. There is nothing in ChronoForms that will change a date format unless you change it.
Are your date inputs free text? That might explain this.

Not sure what you mean by "date inputs free text." If you are referring to the datepicker Field Default Value, it is empty. Let me know if you want to see the admin area.
