Blank page when trying to view form

I am receiving a blank page when I try to view a completed form (http://www.agentismanagement.com/aae_site/index.php?option=com_chronoforms&chronoform=job_board_posting)

I am using Chronoforms V4 (stable_J2.5) on a site using Joomla 2.5.8.

I have included SHOW HTML in my "On Load" Events.

Any idea what I am doing wrong?

Please see this FAQ if talks about a blank page on submit but the same applies if you get a blank page on load - though that is much less common.

I discovered that when I switch to a default Joomla template, the form loads. I am using a template purchased from TemplateMonster.com. When I view my form with the TemplateMonster template in Web Developer tools in Chrome, under Console, I receive this error:

Uncaught ReferenceError: jQuery is not defined
(anonymous function)

Please check that you have the correct version of ChronoForms for Joomla! 2.5 (the Joomla! 2.5 version doesn't use JQuery; the Joomla! 3.0 version may do but Joomla! 3.0 will load it).
