Triple Drop down

Hi, i've purchased the "CF v4 using Ajax" manual in order to make a triple drop-down element in a chronoforms form, but i cannot understand what to add where in order to make it work (not even the example). Where do i put the javascript code? I am not an expert programmer and i would need help with making this work. Is there a way you can tell me step-by-step what to put where?
