Form does not show up

Hi!
I've just installed ChronoForm for Joomla 2.5 so I use the latest version of ChronoForm (and the latest of Joomla for the 2.5.x serie).
After creating the form, I wanted to test it but it doesn't show up. I have the Module title, the Chronoform link signature but that'is it. I've check for javascript error but there are not.
Also, and I do not know if it could be related but, in the backend, even if I click to create the table, it never shows either. I can see in the database that the table for my form has been created correctly but it seems not to be linked to my form!!!

Wow! I did use Chronoform in the past but I did not remember this "subtility". Thank you for the tip! I checked in the forum but I forgot the FAQ. In the meantime, I would respectfully suggest to improve this to be more user friendly such as putting this by default or display a warning. :-)
