Chronoform not displaying :(

The chronoform I added to my website is not showing up on my website at: http://mobility-advice.org.uk/index.php/contact. I am using the {chronoforms}formname{/chrononoforms} tag and the form shows up when I click "Test Form" but does not show up on the website (it only shows the tag chronoforms tag on the website).

I have added a debugger to "Show html" but I'm not sure what it means:

I have looked around for hours for a solution but no luck. I have read something about having mootools installed so I think this might have something to do with it although I can't be certain. Mootools is not installed on my Joomla (2.5) and I have tried to search for it to install but could not find it. will you be able to help me please?

I do not! Do I need all 3 component, module and plugin? I have a component and module installed. I assumed I only needed to choose one of the 3 and originally only had the component (added module later on). I will add the plugin also to see if that solves it then I will report back.

You must have the component installed; you need the module to show forms in template module locations; and the plug-in to include forms in articles.

I am having this same issue and I have the plugin, module, and component installed and enabled... and I have the shortcode displaying on the page. What am I missing?

What do you see when you click the Text Form icon in the Form Wizard - or the Front End View link from the Forms Manager.
