Greetings,

Ok, I created my form - I'm able to Test the form - however, when I link my form to a menu item, the form doesn't show. I have it published under Form settings but its not showing once I click on the menu associated with the form.

Most often this happens because there is no form name added in the Menu setup.

That's the weird part, the name of my form is Service so when creating the menu item, under Basic Options, I entered Service and the only thing that's showing is the Powered By Chronofroms.

Bob, its working now. I guess I needed to empty my history/cache so I could see the updated changes. Thanks for your help anyhow!
