Advanced form is not displayed шт the test mode or modules

joomla 3.0.3
Chronoforms V4 RC3.5.3

I created the form through Form Wizard (not Easy Form Wizard), saved it and tried to test it. As a result, I got a blank screen - no form, only Chronoforms link.

HTML code in the Form Manager is correct.

What am I doing wrong?
