404 on submit, /index.php/{action}

On submit, I land on a 404 and nothing is sent. The form:
http://skotsstix.com/index.php/custom-paddle

Works fine in the "frontend view" from the manager.

This looks like a problem with SEF URLs, please try setting Relative URL to NO on the form General tab. Click the form name link in the Forms Manager link to access it.

Hi Bob and thanks for the response. Relative was already set to NO. I turned off SEF in the global config for the fun of it and when I submit now, I get:

Not Found

The requested URL /{action} was not found on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

I've even tried removing all the actions without success. But it works in the front end view still.
