Please Help me

Dear All,

I created one chrono form and its fine, but i want to link that form with my menu items, when went to add with menu item, I choose chrono display form then it ask a form name, I entered Form Name but when i go to web site and try to Click that menu it shows error

Not Found
The requested URL /travel/frmpatient was not found on this server.
------------------------------------------------

Please help me Asap

Hi Shishir,

This looks like a problem with your SEF URLs or the Joomla! Redirect component if that is enabled. I suggest you check the settings there.
