User Control Panel form

Hi, I am looking for someone to make a form which would allow my sites users to edit & view their website/domain settings.

I have a little hosting site for my friends and basically I just want to offer them the regular settings they should be able to change like: adding/removing email addresses, redirecting domains, parking the domain etc.

I would like the current settings to be displayed (listing the email addresses etc.) Additionally I will need to display data from the table which the user cannot edit for example his "main domain url". However the administrator should be able to edit these fields from the backend.

Edit: Forgot to say that the form is just a way to store and display the information to the users. No actual settings need to be changed, I will do that all manually when I recieve the form emails.
