Changing the 'Load Encoded Image' setting in the Load Captcha action to 'Yes' may resolve the problem.
I changed that option and still gives the same error.
It will show PHP errors on things php cannot handle. This should provide enough input to fix any problem with the server settings..
No error shows me nothing, so I put that link shows is the image that should appear in the captcha.

My best guess is that you don't have the Load Captcha action on that form page.
Please try a simple one page form to test that the Captcha image works OK there.

He was right, I did not know that the "Load Captcha" had to be on the same page as the captcha, but it makes sense.

In general I think it makes most sense to put the Captcha on the first page of a form as it's unlikely that any but the most serious spammer is going to go through four or five pages.
And if your form isn't going to send an email or save data unless all the pages are complete you may not need a Captcha at all.
