I have created a form using wizard. Also I created a menu item as chronoform page and filled up the Form name (name of form I created) and Event (submit as the same button I added to form). The form is published and all setting seems to be fine.
However, when I try to view the page, I don't see anything on the page.
It could be something simple but I don't have a clue...any help would be appreciated.

However I have a minor problem. My form is an online inquiry for private tuition and applicants should tick check boxes which is all fine, however on the body of the email I receive, this is what I get in front of my check box label: (the array bit with bracket etc is extra)
Level array ( 0 => 'A-Level', 1 => 'GCSE', )
Subject array ( 0 => 'Mathematics', 1 => 'Physics', 2 => 'Accounting', 3 => 'Economics', 4 => 'Statistics', )
Can you please tell me how t get rid of this. Much appreciated.
