The reason of Posting this form is to show that CF V4 is a great tool!
This form is completely free of Mootools but full of jQuery (dom manipulation, validations etc)!!! To implement the form I used the posabsolute / jQuery-Validation-Engine (https://github.com/posabsolute/jQuery-Validation-Engine) and fancybox2 lisenced (http://fancyapps.com/fancybox/).
Well the form is in Greek Language but can check out the functionality:
