I have something very strange happening. My Chronoforms v4 form is duplicating on a page. The form was fine, and all of a sudden, we are getting a complete duplication of the form. I have checked the form code, it is not duplicated.
I am using the plugin to publish the form to an article, when I take it out of the page, it completely deletes the form from the page. When I check the Front End View from the Chronoforms Admin, it also shows the form duplicated - see http://www.reynardoptics.com/dev/index.php?option=com_chronoforms&chronoform=quote
Has anyone seen this issue? Any ideas of what has happened. This form was fine until just recently.
