I am not showing this form for any accolades on layout or design, I am posting it to demonstrate the ease and simplicity in which I was able to import 10 custom forms into Chronoforms without a hiccup!
Moreover, I am able to write any submissions to the data base for the client's later use.
Next I plan to convert the submissions to PDF!!
