On my site I am building http://qlxchangestore.com the form seems to work fine however when it send me an email here is all I get

I have been using chronoforms for 4 years and this is my first one with joomla 2.5 and am using Chronoforms_V4_RC3.3_J1.6

am I missing a step to make the details come through in my email? Thanks for any help you can give me

It sounds as thought you may not have added an Email template.

Please rename the 'City, State & Zip' and the 'OneX or OneXpro User Name' without using spaces, commas, ampersands or other characters apart from a-z, 0-9 and underscore e.g name='city_state_zip' and name='username'
