Dear Sirs.

I still haave an issue which seems I can't find the way to solve.
The tick options.

I create a form with options for the "sex" field : (f) (m)
The user choose and tick the box but I don't receive his/her choice with the form.
I receive both of them instead..

Also the telephone..I do not receive the user tel number. I just receive this: {telephone}

The name of the 'sex' input is sex (not male or female); the name of the phone input is phone (not telephone).

You probably want to use radio buttons for sex unless you expect users to choose both male and female.
