I'm looking to create a form for one of my prospects that is similar to the link here:

https://www.cyprexx.com/vendors/vendorapplication.aspx on Step 3. It basically allows you to select multiple states, and available counties based on the selected state. I'm unsure of the proper name for this type of form but my prospect is making this form selection feature a priority.

I've seen this kind of form before but not in ChronoForms. The first two boxes are simple enough - that's a double drop-down with a slightly different layout for the second. The linking of the second and third boxes is done with JavaScript linked to the buttons. That must exist somewhere on the web already and would be not too bad to write from scratch (but not trivial either).
