Send Copy to All Addresses Entered in Form

I have a form where the user has to fill in a couple email addresses, their manager, supervisor, etc... How can I get the form to be sent to the email addresses that were entered?

You can add them to a Dynamic CC in the Email action. I'm not sure of the exact syntax for adding more than one - there was a thread talking about multiple addresses in the Dynamic To box a little while ago.
