mod_chronoforms form name parameter

I've read that for displaying a form in a module is necessary to create a mod_chronoforms module. I do not see any place where to input form name. I have no right column. Is this a bug? Am I doing something wrong?

You need to download and install the ChronoForms module from the Downloads area here.
