Chronoforms v4 - How to populate a select box in a form?

I am unable to populate the select box in the form I have created. It will only hold to first or last value. I have about 20 options I need listed. Please Help Me!!!

What do you have in the Options box?

Business Categories like,

Apartments
Auto Sales
Book Stores
Car Rental Services

Etc.

The format is name=value so I think you need to have

Apartments=Apartments
Auto Sales=Auto Sales
Book Stores=Book Stores
Car Rental Services=Car Rental Services
