File upload fails to attach to email

I have searched all the existing posts on this and found nothing that has helped me.

My form sends fine, but the attachement is not attached.

I have enabled and un-enabled file up load, checked the path, tried various permissions on the folder . . . on and on. Nothing has helped.

Here is the latest debug:

Hi tjhaas ,

It looks as though you may need to put file1 in the Attachments box on the Email configuration.
