Any idea what we need to do to get the contact form to display on this page acupuncturebymarks.com/contact?

Form is here acupuncturebymarks.com/index.php?option=com_chronocontact&chronoformname=contact_form

Just transferred hosts and can't get it to work.

Got it. Had to reenable "chronocontact" plugin.

It should work with a Menu item of type ChronoForms or External URL
