on my site I have a form for register books On the site the order is: School name; School address; City and State (in one field); Zip, etc. But in the emails I get when someone registers a book, after Where you Purchased Book(s) From, it says School Name; then City but then gives the School Address. example below

This does't tell me enough to know what the problem is. I can only suggest that you go back and check the email template against the form inputs and see what the difference is. Perhaps you have some duplicaed input names?
