Ok. After a few hours of trial and error here is what I got. I'm close.
Also, might help others to see screenshots.

Page 1 - Enter Company, enter Program checkbox values. Two fields.
Page 2 - Email and Name Fields (With summary from Page 1). Two fields.
Page 3 - Summary Form (Page 1 & Page 2). Four fields.

After confirming Page 3/Summary Form, user submits and email is sent information to me and user.

Page 2 - Checkbox values not applied. Program field reads "Array" instead.

Page 3 - Checkbox values are summarized correctly.

Email - Field data is not populated. Email template and final email look the same. Curly brackets and all.
