Can anyone tell me what happen here:

Please help ... this form is life on website and customer is bit angry form is not working properly Thank you!

It looks as though there is a problem with some PHP in the Email template box. What do you have in there?

Thank you for your quick response, indeed it was in the email template ... there was some php code inside ... I removed it and now it works normal again ...
