Unknown validation error preventing form submission

We have a form which upon submission shows a validation error...see the attachment. As you can see no error is shown; submission of the form does not happen; there are no emails sent.

What can I check that could cause this?

Is this the latest ChronoForms version 3.2?

If so there's a bug in the File upload code. Please see this post.

Yes, 3.2. I applied the new PHP page and it took care of it. Thank you.
