Identify you registered on Form

I want to create a form for user registration completion and sending photographs and other data. So far so good. The question is: How to receive data identifies the form in the mail that comes from a given user without the need to fill fields with name and surname / company? Must create some special field?

Are these registered users? If they are then you can get their name and email from the Joomla! User Object.
